Getting Started - New CogicCard Members

 

Step 1 Fill out Application:

  • Click on >Application, go to >Online Application and fill out the required information or,
  • visit www.primefinancialcu.org, click on the picture of the CogicCard and go to > Online Application in the box outlined in red.  Print this form, fill out and sign or,
  • Ask your church’s CogicCard Coordinator or,
  • Call 1-866-75-COGIC

 

 

Step 2 Pick a Package that Fits your Needs:

Package 1 - $ 35.00 ($ 55.00 for joint account) which includes:

• $20.00 one-time processing fee per card

• $15.00 in your account of which $5.00 remains in your savings account for membership to the credit union

Package 2 - $ 50.00 ($ 70.00 for joint account) which includes:

• $20.00 one-time processing fee per card

• $30.00 in your account of which $5.00 remains in your savings account for membership to the credit union

Package 3 - $100.00 ($120.00 for joint account) which includes:

• $20.00 one-time processing fee per card

• $80.00 in your account of which $5.00 remains in your savings account for membership to the credit union

You are not limited to the above amounts.  Any money that exceeds the package amount will be deposited into your Prime Financial account!

Step 3 Submit Application 

  • Send your application, copy of current I.D., and deposit to:

Cudahy, WI 53110 or,

• Give your signed application, current driver’s license or state I.D., and initial deposit to your church’s CogicCard coordinator

 

Getting Started - Existing CogicCard Members

Step 1 Fill out Application/Verification Form:

  • Click on >Application, go to >Online Application and fill out the required information or,
  • Visit www.primefinancialcu.org, click on the picture of the CogicCard then click on >Online Application in the box outlined in red.  Print this form, fill out and sign, or
  • Ask your church’s CogicCard Coordinator or,
  • Call 1-866-75-COGIC
 

Step 2 Submit Application/Verification Form

  • Send your application/verification form* to:

CogicCard Member Services

5656 S. Packard Ave.

Cudahy, WI 53110 or,

  • Give your signed application/verification form to your church’s CogicCard coordinator

You are not required to send a copy of a valid ID or submit any money at this time.

*  If you would like to send an inital deposit for your CogicCard account with your updated application/verifcation form, please feel free to do so.

Getting Started - Cogic TEENCard Members

Step 1 Fill out Application:

  • Click on >Application, go to >Online Application and fill out the required information or,
  • Visit www.primefinancialcu.org, click on the picture of the CogicCard and then click on >Online Application in the box outlined in red.  Print this form, fill out* and sign** or,
  • Call 1-866-75-COGIC

*  The teen will be the primary card holder and a parent or guardian must fill out the joint/supplemental section.  Your parent or guardian must be a CogicCard holder.

** Both teen and Parent or guardian must sign this form.
